Orforglipron is a first-in-class oral, non-peptide GLP-1 receptor agonist developed for metabolic disease and weight management. Unlike injectable GLP-1 medicines, orforglipron is a small molecule taken once daily by mouth.
It is now marketed in the United States as Foundayo and received FDA approval in April 2026 for chronic weight management in adults with obesity or overweight accompanied by at least one weight-related comorbidity.
Product Information
Orforglipron activates the GLP-1 receptor, producing effects that include increased glucose-dependent insulin secretion, reduced glucagon signaling, delayed gastric emptying, and increased satiety.
Phase 3 research demonstrated clinically meaningful weight reduction with once-daily doses of 6, 12, and 36 mg in investigational regimens.
How Does It Work?
Orforglipron activates GLP-1 receptors involved in glucose regulation and appetite control. This can increase satiety, reduce appetite, slow gastric emptying, and improve glucose-dependent insulin secretion.
Benefits
Supports Significant Weight Reduction
Phase 3 trials demonstrated meaningful reductions in body weight, with higher investigated doses producing greater average weight loss than placebo.
Helps Control Appetite
GLP-1 receptor activation influences appetite-regulating pathways, helping reduce hunger and food intake as part of a structured weight-management program.
Improves Glycemic Control
Clinical research in adults with type 2 diabetes found that once-daily orforglipron improved HbA1c while also reducing body weight.
Convenient Oral Administration
One of orforglipron’s biggest differentiators is its oral, once-daily format. It does not require injection and can be taken with or without food.
Side Effects
Common adverse effects are primarily gastrointestinal and can include nausea, vomiting, diarrhea, constipation, abdominal discomfort, and reduced appetite. More serious warnings include pancreatitis, severe gastrointestinal reactions, dehydration-related kidney injury, and hypoglycemia when used with insulin or insulin secretagogues.
The FDA label also carries a boxed warning concerning thyroid C-cell tumors and contraindicates use in individuals with a personal or family history of medullary thyroid carcinoma or MEN2.
Dosage
The current U.S. prescribing information starts Foundayo at 0.8 mg once daily. After at least 30 days, the dose can increase to 2.5 mg, then 5.5 mg, followed by 9 mg, 14.5 mg, or 17.2 mg depending on response and tolerability. The maximum labeled dose is 17.2 mg once daily.
