Clomiphene is a selective estrogen receptor modulator (SERM) with a long history in reproductive medicine. Its primary established indication is ovulation induction in women, but clomiphene has also been extensively investigated off-label in men with low testosterone.
Rather than supplying testosterone directly, clomiphene works upstream by influencing estrogen feedback at the hypothalamus and pituitary.
Product Information
Clomiphene blocks estrogen feedback at estrogen receptors in the hypothalamic-pituitary system. This can increase secretion of LH and FSH, stimulating endogenous gonadal hormone production.
In men, clinical studies have reported increases in testosterone following clomiphene treatment, although male use remains off-label.
How Does It Work?
By reducing estrogen-mediated negative feedback, clomiphene can increase GnRH signaling and subsequently increase LH and FSH secretion. In women, this stimulates follicular development and ovulation. In men, increased LH can stimulate endogenous testosterone production.
Benefits
Supports Ovulation
Clomiphene is an established oral fertility medication used to stimulate ovulation in appropriately selected women.
Raises Endogenous Testosterone in Some Men
Clinical studies have found that clomiphene can increase testosterone in men with hypogonadism while stimulating the body’s own hormonal axis.
Preserves Endogenous Hormonal Signaling
Because clomiphene stimulates the hypothalamic-pituitary-gonadal axis rather than supplying exogenous testosterone, it has attracted interest where preservation of endogenous reproductive function is important.
Well-Established SERM Pharmacology
Clomiphene has decades of clinical experience and a clearly characterized estrogen-receptor mechanism.
Side Effects
Possible side effects include hot flashes, headache, mood changes, nausea, breast tenderness, visual disturbances, and ovarian enlargement in women. Visual symptoms can be particularly important because persistent or severe visual changes warrant medical evaluation.
Dosage
For its FDA-approved female ovulation-induction indication, the labeled starting regimen is 50 mg daily for 5 days. If ovulation does not occur, the label allows a second course of 100 mg daily for 5 days, with specific limits on repeated treatment.
Male clinical studies have commonly investigated 25 mg daily or 25 mg every other day, but male testosterone treatment is off-label.
